HomeCare Maryland is a Medicare-certified and Joint Commission-accredited provider of home health services. Our staff members undergo extensive background checks to ensure you receive the highest quality care. Your team of healthcare professionals utilizes an Electronic Health Record (EHR) to ensure each visiting staff member has quick and easy access to your medical record and can collaborate with your physician and other healthcare providers with ease. Once your doctor refers you for home health services, our team will schedule an appointment and come to your home to discuss your needs and develop an individualized care plan for you. We also maintain communication with your doctor regarding your progress and provide them with regular updates throughout the process. HomeCare Maryland ensures seamless continuity of care.

Accepting patient referrals 24 hours a day, 365 days a year

 

We accept new referrals 24 hours a day, 365 days a year and a nurse is always available by phone. HomeCare Maryland serves patients in Baltimore City and Baltimore, Frederick and Carroll counties. Anyone can refer patients to HomeCare Maryland and patients can self-refer if needed. To start the referral process, select the type of referral needed and complete the correct paperwork. Once completed, submit the necessary paperwork to referrals@homecaremaryland.com.

    Registered Nurse (RN)

    We are growing and have multiple territories open! HomeCare Maryland is looking for Full-time/Part-time/PRN Registered Nurses. At HomeCare Maryland we offer a comprehensive benefits package to eligible employees. 

    Position Summary : Responsible for providing professional nursing care to patients following established standards and practices in the home setting. Functions as primary clinical liaison between patients and physicians.

    Essential Functions

    • Performs initial and ongoing assessment of physical, psychosocial and environmental needs of patients in the home setting
    • Confirms assessment findings with physicians and completes documentation
    • Develops therapeutic nursing interventions to meet patient's individual goals and determines the need for additional skilled services
    • Provides supervision for home health aide and licensed practical nurses as appropriate per the policies of HomeCare Maryland, LLC and the Maryland regulatory agency

    Qualifications/Requirements: 

    • Current-valid Registered Nurse license for the state of Maryland
    • Current American Heart Association BLS for healthcare providers
    • Prior experience in home care preferred, but not required...positive attitude a must! 
    • 2-3 years as a Registered Nurse
    • 2-3 years acute care setting experience

       

    Admissions Nurse

    Summary of Position: Responsible for providing professional nursing care to patients following established standards and practices in the home setting. Functions as primary clinical liaison between patients and physicians. Completes admission visits, OASIS documentation and other documentation as needed for new and existing patients.

    Essential Functions

    • Completes the admission process appropriately for skilled services per regulatory standards and per the policies and procedures of HomeCare Maryland. Completes thorough and accurate OASIS data and plan of care
    • Performs initial and ongoing assessment of physical, psychosocial and environmental needs of patients in the home setting
    • Confirms assessment findings with physician at the start of skilled services
    • Develops therapeutic nursing interventions to meet patient's individual goals and determines the need for additional skilled services
    • Provides supervision for home health aide and licensed practical nurses as appropriate per the policies of HomeCare Maryland, LLC and the Maryland regulatory agency

    Education and Experience:

    • Registered Nurse License for the state of Maryland
    • Current BLS for Healthcare Providers
    • Prior experience in home care preferred, positive attitude a must!
    • 2-3 years as a Registered Nurse
    • 2-3 years acute care setting experience

    Scheduler

    Summary of Position: The Scheduler is responsible for coordinating all patient care visits per
    Medicare, State and Federal regulations. This position is also responsible for clerical duties
    including, data input, maintenance of patient electronic medical records, tracking outstanding
    documentation, answering multi-line office phones and scheduling visits for all patients.
    Essential Functions:

    • Maintains ongoing scheduling of patient visits for all clinicians based on physician orders
      and insurance requirements
    • Assigns clinicians and schedules visits based on patient needs, staff experience and
      location
    • Assigns appropriate Oasis tasks as needed and monitors for completion
    • Monitors clinicians’ schedules to maintain a productive workload
    • Monitors scheduling reports to ensure schedule and visit accuracy
    • Communicates with Intake daily to transition patients to home care or resume services


    Education and Experience:

    • High School Diploma
    • 1 year scheduling experience, prior home care experience preferred
    • Experience using Microsoft Office
    • Experience with electronic medical records (EMRs)
    • Great customer service experience
    • Knowledge of Medicare rules and regulations
