BridgingLife

This includes support for family members and other caregivers as they care for loved ones, including connection to community resources to ensure continuity of care as well as grief support. 

HomeCare Maryland 
HomeCare Maryland is a Medicare-certified and Joint Commission-accredited home health agency that sends experienced care providers to patients who are homebound, even temporarily, in an effort to enhance well-being and recovery from home. Service providers include physical therapists, occupational therapists, skilled nursing professionals, speech language pathologists, medical social workers and home health aides, who are specially trained to provide personalized care for unique medical conditions.
