Impacting Community Health for Today and the Future

At LifeBridge Health, we care about our communities. That is why we are determined to focus on health and wellness initiatives that offer a positive impact. We strive to understand and improve the health outcomes of populations through a value-based care environment that offers a holistic, data-driven and integrated approach to the communities we serve.

 

We partner with community leaders, municipalities and other health systems to proactively identify and address the social determinants of health impacting our communities. We seek to influence our region’s policy dialogue that allows health systems to optimally support the health and well-being of our community.

Programs to Support You and the Community

LifeBridge Health is dedicated to offering a variety of community initiatives and programs for men, women, and families. With special support from the M. Peter Moser Community Initiatives Funds, we offer non-medical programs designed to improve the social well-being of our communities.

In partnership with the Family League of Baltimore, Sinai Hospital delivers Healthy Families America (HFA) home visiting services from pregnancy through the first 3 years of a child's life, linking families with essential health and social support. LifeBridge Health offers regular Community Health Needs Assessments (CHNA) to help make informed plans that address high priority needs and better engage communities to understand their health knowledge, conditions, behaviors, and general needs, such as those related to chronic disease prevention and management, and ability to access health care.
