The Healthy Hearts: Cardiovascular Disease Prevention Project is a risk reduction initiative to reduce health disparities and advance health equity in the Sandtown-Winchester community.
To support that mission, we provide advocacy, education, support, and public awareness around mental health issues.
Grace Hospital-based Violence Response Team (VRT) is comprised specifically of trained individuals who assist patients at bedside experiencing trauma/and or injury as a result of an act of violence with resources to improve their health and well-being. VRT is a 24/7 crisis response program designed to assist those who are victims of domestic violence also known as intimate partner violence, community violence, vulnerable adult abuse and elder abuse. Our goal is to ensure safety, offer emotional support and connect patients to comprehensive community-based services and resources.
Our team is focused on assisting members of the community to learn about and how to manage their health, connecting them to resources at Grace/Lifebridgehealth and resources in the community. We are also a referral resource for the community. Ultimately, we assist the community to find a medical home at Grace Medical Center and help to reduce inappropriate hospital admissions, which drives higher health care costs.
We're able to assist individuals requesting services, with information and locations to obtain the services within the surrounding communities.
