A Seamless Continuum of Care from Hospital to Home

At LifeBridge Health, our Virtual Care Team is here to provide you with essential support services, including post-discharge care, remote patient monitoring and Annual Wellness Visits (AWVs). These virtual services are designed to help you stay connected, supported and healthy from the comfort of your home.  

Post-Discharge Support

When you leave a LifeBridge Health Hospital or Emergency Department, you will be automatically enrolled on a digital patient engagement, education, and communication tool called Get Well Loop. Get Well Loop allows you to engage with virtual healthcare providers through automated virtual check-ins and the ability to send and receive comments, ensuring a smooth transition from hospital to home.

Remote Patient Monitoring

You may qualify for remote patient monitoring services if you have recently been discharged from a LifeBridge Health facility with a chronic condition like hypertension, diabetes, congestive heart failure, COPD and more. Our secure platform is designed to cater exclusively to the unique needs of patients managing these chronic conditions, and it is accessible only to those patients who have received enrollment invitations from one of our nurses. 

 

Through advanced monitoring technologies, we provide continuous and personalized oversight, allowing our patients to take control of their health from the comfort of their homes. Our dedicated team of healthcare professionals is committed to delivering real-time insights, fostering preventive care and ensuring that every individual receives the attention and support necessary for effective chronic disease management. 

Medicare Annual Wellness Visits (AWV)

Dedicated to ensuring your well-being, our Virtual Care Team conducts AWVs for several primary care providers, streamlining the process and enhancing accessibility. Through thorough assessments and personalized consultations, we prioritize preventive care, helping you proactively manage your health.
