Notice to Our Patients of Privacy Incident
LifeBridge Health (“LifeBridge”) is committed to the privacy and security of our patients’ information. Regrettably, this notice is regarding an incident that may have involved unauthorized access to certain patients’ health information.
On November 12, 2024, LifeBridge determined that an unauthorized third party accessed a limited number of employees’ email accounts from August 27 to September 21, 2024, and may have obtained some patient information. The incident did not affect all LifeBridge patients.
We conducted a detailed review and analysis of the contents of the email accounts to determine what and whose information was included. The information may have included patients’ names and one or more of the following: dates of birth, dates of service, Social Security numbers, medical record numbers, Health Insurance Claim Numbers, and limited treatment information. Patient electronic health records were not involved.
We began notifying affected patients on January 10, 2025, and as a precaution, are offering eligible patients complimentary credit monitoring and identity protection services. We recommend that patients review the statements they receive from their health insurer or healthcare provider. If they see charges for services they did not receive, the patient should contact the insurer or provider immediately.
To help prevent an incident like this from happening again, LifeBridge continues to provide privacy and cybersecurity training to our staff and identify additional safeguards to enhance the security of our email environment.
