Outpatient Mental Health Clinic at Grace Medical Center

Grace Medical Center’s Outpatient Mental Health Program (OMHP) provides services for depression, anxiety, trauma and other mental health needs. Our services include counseling for individuals, groups, couples and families, medication management and referral services for anyone who struggles with substance abuse.

The behavioral health team at Grace Medical Center believes in the connection between mind, body and spirit. As part of treatment, we address those areas and help patients move toward their mental health recovery.

We also recognize and appreciate each individuals’ unique characteristics and personalities as we build alliance for the healing process.

The OMHP provides quality mental health services for adults, adolescents, children and support for families. Our experienced staff helps our patients manage symptoms and provide direction towards a healthy and productive lifestyle.

Mental health treatment is used in conjunction with psychiatric and medication support services at our facility.

 

Intensive Outpatient Program
Grace Medical Center’s intensive Outpatient Program (IOP) offers intensive, outpatient psychiatric services to patients who require less than 24-hour care, but who need more comprehensive psychiatric evaluation and treatment than can be provided in a traditional outpatient setting. Below, learn more about what our pediatric and adult IOP programs provide.

 

For Adults:

  • Three hours of treatment
  • Individual assessment
  • Medication consultation and evaluation
  • Group psychotherapy treatment
  • Individualized treatment planning and scheduling
  • Support in discharge planning *Length of stay is always determined by individualized needs and response to treatment.

For Children:

  • Six hours of treatment
  • Individual assessment
  • Medication consultation and evaluation
  • Group, family and individual psychotherapy treatment
  • Individualized treatment planning and scheduling
  • Support in discharge planning *Length of stay is always determined by individualized needs and response to treatment.

 

Partial Hospitalization Program
The Partial Hospitalization Program (PHP) at Grace Medical Centeris a structured, intensive day treatment program for adults who are in need of psychiatric stabilization.

PHP is a culturally sensitive, therapeutic program facilitated by a multidisciplinary team of psychiatrists, licensed therapists, social workers, registered nurses and behavioral health technicians who are committed to providing quality health care and empowering patients.

Individualized treatment focuses on relief of acute symptoms, establishment of healthy coping skills and collaboration with family and community support resources.

Outpatient Mental Health Clinic at Levindale

The mission of the Outpatient Mental Health Clinic at Levindale is to provide compassionate treatment for the emotional and psychological needs of older adults, usually age 50 or over.

 

Being emotionally and mentally healthy enhances overall well-being, improves quality of life and maximizes a person's ability to cope when faced with life's challenges. The intent of our programs is to provide mental health therapies to individuals so they can maintain a healthy outlook, whether they experience chronic or new-onset psychiatric or behavioral disorders.

 

Reasons to seek assistance include:

  • Changes in mood or behavior
  • Decline in memory
  • Isolation from family or friends
  • Excessive feelings of guilt, helplessness or hopelessness
  • Frequent or uncontrollable crying
  • Sleep disturbance

Mental health treatment methods include:

Referrals
Referrals are welcome from physicians, health care providers, residential facilities, family members and other involved people. Please call 410-601-2358 to refer a patient or to refer yourself for evaluation.

The Outpatient Mental Health Clinic is a participating Medicare provider.

Intensive Outpatient Program (IOP)
Are you or a loved one ready to be part of the community but still in need of support? Our Intensive Outpatient Program (IOP) provides transitional care to adolescents who can function more independently.

 

It includes:

  • Three hours of clinically intensive care
  • Psychiatric assessment, evaluation and consultation
  • Group and family therapies
  • Medication monitoring and education
  • Therapeutic intervention designed to help you regain the confidence to pursue a healthy, productive life
  • Discharge planning coordinated with community resources to enhance successful recovery and return to community
  • Special programs tailored for adults and adolescents ages 13 to 18; the treatment and care of each group is provided in separate areas.

 

Dual Diagnosis Component
We recognize that many people suffer from underlying behavioral health conditions that lead them to abuse alcohol or drugs. Our Dual Diagnosis Program provides effective outpatient treatment or partial hospitalization to help you or a loved one deal with the double challenge of substance abuse and a behavioral health problem.

 

Our professional, experienced staff provides:

  • Concurrent treatment for mental health and addictions disorders
  • Psychiatric assessment, evaluation and consultation
  • Group and family therapies
  • Medication monitoring and education
  • Substance abuse treatment and education
  • Discharge planning coordinated with community resources to enhance successful recovery and return to community

 

We have programs tailored for adults and for adolescents ages 13 to 18; the treatment and care of each group is provided in separate areas.
