Hospital and physician charges

Medical bills for hospital services can be difficult to understand. For most hospital services, patients can expect to receive two bills, one for hospital services and one from each physician or provider who provided care during the hospital stay.

Certain physician or outpatient clinics located in the hospital are designated and considered part of the hospital. These locations are listed below and represent hospital-based clinics where patients may receive a hospital and physician bill separately.

What is facility fee or “hospital-regulated based” billing?

Facility fee or hospital-regulated based billing is a status defined by the State of Maryland Health Services Cost Review Commission (HSCRC) and the Centers for Medicaid and Medicare Services (CMS) that refers to the billing process for services rendered in a hospital outpatient location. Patients seen at these locations may receive bills for hospital and physician services, resulting in two bills.

Why are facility fees charged?

How will my bills be different at hospital-regulated based locations?

In most cases, charges for services at hospital-regulated based locations are split apart for professional services (physician and other provider fee) and hospital services (facility fee). Depending on insurance coverage, it is possible benefits will be different and some patients may have to pay a higher cost because of hospital charges (facility fees). Patients are encouraged to contact their health insurance plan for more information about insurance coverage and benefits. Patients with Medicaid or a Medicaid replacement plan have no out-of-pocket costs at hospital-regulated locations. Also, Medicare patients with supplemental insurance may not have any out-of-pocket costs.

Can you provide more details about the billing process?

Patients not covered by Medicaid or Medicare with secondary insurance may receive two separate bills for services provided at the locations listed above. One bill will be for professional services delivered by the physician and other providers, and a second bill for hospital facility fees. Once insurance processes their portion of the claims, the balance will be submitted to a secondary insurance plan, if one is provided. Patients who have a balance after the secondary insurance pays the claim or who do not have secondary insurance coverage may receive two bills for the remaining balance.
