With the LifeBridge Health Patient Portal, your health information is available 24 hours a day, seven days a week via your computer, tablet or smartphone.
Medication Refill
Need a refill? Contact your pharmacy first—they’ll coordinate with our office to process your request. To avoid running out, request a refill when you have at least a week’s supply left. You can also reach out to our office via the patient portal for prescription refill requests.
Insurance and Referrals
We accept most major insurance plans. Check with your provider or call our office to confirm coverage. If your insurance requires a referral to see a specialist, we’re happy to help. Please allow 24 hours for processing.
Fax referrals to the specialist or pick them up in our office. Don’t forget to bring your insurance card to every visit!
