An electronic personal health record (PHR) allows you to keep important medical information and notes on care you have received, including lists of medications and dates of doctor and hospital visits.
The LifeBridge Health MySELF Personal Health Record is a safe and secure way to stay on top of all your medical information. You are in complete control of you record. You can decide who has access to your record and what information to store in it.
