TRANSITIONS OF CARE MEDICINE

The Digital Care Center bridges the gap between hospital discharge and continued healthcare support through transitions of care telemedicine services. When you leave a LifeBridge Health hospital or emergency department, you will receive an email or text invitation to our GetWell Loop portal. This link seamlessly connects you with virtual healthcare providers, ensuring a smooth transition from hospital to home.

By leveraging advanced telemedicine technologies, we empower patients to receive ongoing medical attention, monitor their recovery and address any concerns – all from the comfort of their homes. Our mission is to enhance post-discharge care, reduce hospital readmissions and ultimately improve the overall well-being of our patients through accessible and personalized virtual healthcare.

REMOTE PATIENT MONITORING

You may qualify for remote patient monitoring services through the Digital Care Center if you have recently been discharged from a LifeBridge Health facility with a chronic condition like hypertension, diabetes, congestive heart failure, COPD and more. Our secure platform is designed to cater exclusively to the unique needs of patients managing these chronic conditions, and it is accessible only to those patients who have received enrollment invitations from one of our nurses.

Through advanced monitoring technologies, we provide continuous and personalized oversight, allowing our patients to take control of their health from the comfort of their homes. Our dedicated team of healthcare professionals is committed to delivering real-time insights, fostering preventive care and ensuring that every individual receives the attention and support necessary for effective chronic disease management.

EVISITS

eVisits revolutionize the traditional healthcare model by enabling patients to receive quick diagnoses and personalized treatment plans for common conditions like colds, allergies, rashes, ear infections and more. Patients can engage with healthcare providers asynchronously through secure and user-friendly interfaces, eliminating the need for real-time interactions. This innovative, self-guided approach allows individuals to share their symptoms and seek medical guidance on their own schedule, making healthcare more accessible and responsive.

MEDICARE ANNUAL WELLNESS VISITS (AWV)

Dedicated to ensuring your well-being, the Digital Care Center conducts AWVs for several primary care providers, streamlining the process and enhancing accessibility. Through thorough assessments and personalized consultations, we prioritize preventive care, helping you proactively manage your health.

VIRTUAL NURSING

Virtual nursing is a collaborative effort designed to enhance patient care within our facilities. The program bolsters in-person care by giving patients real-time access to virtual nurses employed by LifeBridge Health’s Digital Care Center. Virtual nurses, who communicate via iPad, serve as invaluable partners, providing assistance, expertise and support to their on-site counterparts.

Our team recognizes how meaningful your relationship with your bedside nurse is. We aim to strengthen this relationship by handling supplementary services virtually, including education and planning, so your bedside nurse can fully focus on bedside and touch-oriented care. This dynamic collaboration ensures a holistic and responsive approach, where virtual and bedside nurses work hand in hand to address patient needs around the clock.
