Create your Username and Password (Save this information for recertification purposes) |

Create your Security Question and Answer |

Enter your name |

Enter your email address |
| 6. |
CME/CEU credits (required) |
| 7. |
Course Survey (required) |
| 8. |
Click on SUBMIT |
| 9. |
Enter Member Information |
| 10. |
Click on SUBMIT |

Scroll down to CITI Course Enrollment Questions |
| 12. |
Select the group appropriate to your research activities. |
| 13. |
Click on SUBMIT |
| 14. |
If participating in research at another institution, you may click "Yes" to select that institution. Otherwise, click "No". |
| 15. |
Scroll down to "My Courses" and click on the word "Enter" under "Status". This will bring you to the Required Modules. |

Once all modules are completed, you will be give the option of printing out the report.
